COVID-19 Screening

Before entering the Training Center, you must answer the brief quiz below.

Per our COVID-19 training plan, you must take this brief screening test within 24 hours before entering our building. Please note: this screening is NOT a COVID-19 test & DOES NOT determine whether or not you are infected with or have been exposed to COVID-19.

If you hesitate to answer any of these questions in a positive manner, please call us at (763) 533-5514 to reschedule your training. We will make every effort to work with you to keep all trainees safe and current on their training. Thank you.

If you pass this screening, you will see a green checkmark. Show us the green checkmark on your phone when entering the building to proceed to your temperature check. You will also receive an email with a copy of the green checkmark. You can also print or show us an electronic copy of that document to proceed.

If necessary, paper copies of this screening are available on request.

Health Quiz

1. In accordance with Local 539's Joint Apprenticeship & Training Committee's COVID-19 Plan for the Training Center:
    • The number of visitors to the Training Center will be limited to only those necessary for the work.
    • All visitors will be screened in advance of arriving at the training center. If the visitor fails the screening quiz or has a high temperature reading, he/she should not be permitted to access the training center.
2. Have you been confirmed positive for COVID-19, or are currently waiting on results of a COVID-19 test?
3. If you DID test positive, have you had a negative test result or been cleared by your medical provider since then?
4. Are you currently experiencing, or recently experienced, any acute respiratory illness symptoms such as fever, cough, or shortness of breath?
5. Within the past two weeks, have you been in close contact with any persons who has been confirmed positive for COVID-19?
6. Within the past two weeks, have you been in close contact with any persons who have traveled and are also exhibiting acute respiratory illness symptoms?
7. ALL VISITORS MUST COMPLY With THE COVID PRE-SCREEN & TEMPERATURE CHECK. FURTHER ACCESS MAY BE DENIED PENDING THE RESULTS OF THE SCREENING.
8. WEARING OF FACE COVERING WILL BE REQUIRED.
9. YOU ARE NOT REQUIRED TO PROVIDE THIS DATA. HOWEVER, IF YOU REFUSE TO PROVIDE THE DATA, YOU WILL NOT BE ALLOWED ACCESS TO THE TRAINING CENTER.
